29 CFR 1926.453(b)(2)(v): A body belt was not worn and/or a lanyard was not attached to the boom or basket when working from an aerial lift. On or about December 4, 2023, on the exterior of a commercial building, employees were exposed to fall hazards while accessing the roof from an aerial lift without a fall protection system.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On or about December 4, 2023, at the jobsite, where employees were exposed to fall hazards exceeding 6 feet while installing glass and metal framing on the roof of a commercial building without a fall protection system. The Advanced Glass Solutions LLC was previously cited for a violation of this Occupational Safety and Health Standard or its equivalent standard CFR 1926.501 (b)(1) , which was contained in OSHA inspection number 1585586, citation number 1, item number 1 and was affirmed as a final order on 6/23/2022, with respect to a workplace located at 3615 Grand Parkway North, Katy, TX 77449.
