29 CFR  1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes.  a)	On or about December 1, 2023, at a jobsite located at 7680 Ottawa Road. Cairo, OH: subcontractor employees performing electrical work activities atop the 1st deck mezzanine (metal grate platform) of a commercial structure were exposed to the hazard of falling through a floor opening.  The controlling employer, Industrial Turnaround Corporation dba ITAC, did not ensure that the subcontractor employee(s)  were adequately protected from hazards of falling approximately 15 feet to the ground, through floor opening while installing electrical control panels in close proximity of the uncovered floor opening.
