29 CFR 1926.501(b)(4)(i): Each employee on walking/working surfaces was not protected from falling through holes (including skylights), more than six feet (1.8 m) above lower levels, by personal fall arrest systems, covers, or guardrail systems erected around such holes. a) RMF Nooter LLC. on or about December 1, 2023 at the address of 7680 Ottawa Road. Cairo, OH 45820: The employer failed to ensure employees performing electrical work activities atop the 1st deck mezzanine (metal grate platform) of a commercial structure were adequatly protected from falls through a floor opening. Employee(s) installing electrical control panels in proximity of the uncovered floor opening were exposed to falls of 15 ft. to the concrete floor below.
