29 CFR 1910.28(b)(3)(ii): Protection from fall hazards - Holes. The employer must ensure: Each employee is protected from tripping into or stepping into or through any hole that is less than 4 feet (1.2 m) above a lower level by covers or guardrail systems; a) On or about November 28, 2023, the employer did not ensure that employees walking on the Line 7 Oven room floor during machine cleaning and sanitation were protected from stepping into uncovered floor drains that measured approximately 13 inches deep, exposing employees to trip and fall hazards.
