29 CFR 1910.178(a)(4): Modifications or additions which affect capacity and safe operation of powered industrial truck were performed by the employer without the manufacturer's prior written approval:   a) Northern Pelagic Group, LLC- On or about December 5, 2023, employees operated reach trucks with additions, a forklift aerial platform to conduct inventory, and the employer did not obtain the manufacturer's written approval before use, exposing employees to tip-over hazards. 29 CFR 1910.178(l)(3)(i)(G): Powered industrial truck operators did not receive initial training in fork and attachment adaptation, operation, and use limitations:  a) Northern Pelagic Group, LLC, located in Hazleton, PA- Employees were exposed to a fall hazard from approximately 20 feet to the concrete ground level below, while conducting inventory from a forklift platform that had an unprotected side, three sides with rails that were not 42 inches plus or minus 3 inches, no fall protection measures in use and employees were not trained on forklift attachment and use limitations.  Abatement certification required.
