29 CFR 1910.303(b)(1):Examination. Electric equipment shall be free from recognized hazards that are likely to cause death or serious physical harm to employees.   a. On or about December 5, 2023, the employer did not examine, identify, and correct damaged exit signs located at the end of aisles 4, 5, 7, 11, and 28. Signs were suspended by their conductors and not fastened to the junction box.   Reliable Healthcare Logistics LLC was previously cited under the name Woodfield Distribution LLC for a violation of this Occupational Safety and Health standard 29 CFR 1910.303(b)(1), which was contained in OSHA inspection number 1615205, citation number 1, item number 3a, and was affirmed as a final order on May 26, 2023, with respect to a workplace located at 5653 Creekside Parkway, Suite B, Lockbourne, OH 43137.

29 CFR 1910.303(b)(8)(i):Electric equipment shall be firmly secured to the surface on which it is mounted.  a. On or about December 5, 2023, the employer did not ensure that the electrical outlet box used to energize the Hawker, Model LMP3-48F-320Y forklift charger was secured to the wall. The 480 volt box was only supported by the metal conduit and connector fitting.   Reliable Healthcare Logistics LLC was previously cited under the name Woodfield Distribution LLC for a violation of this Occupational Safety and Health standard 29 CFR 1910.303(b)(8)(i), which was contained in OSHA inspection number 1615205, citation number 1, item number 3c, and was affirmed as a final order on May 26, 2023, with respect to a workplace located at 5653 Creekside Parkway, Suite B, Lockbourne, OH 43137.
