29 CFR  1926.1053(b)(1):Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend to at least 3 feet (.9m) above the upper landing surface to which the ladder was used to gain access nor was the ladder secured at the top to a ridged support and a grasping device, such as a grabrail, provided to assist the employee in mounting and dismounting the  ladder:   a) 120 Passaic Street, Hackensack NJ:  An employee was allowed to use a 25-foot Werner Multi position pro ladder to access the roof. The ladder side rails did not extend at least 3 feet above the upper landing, on or about 9/19/23.

29 CFR 1926.1053(b)(5)(i): Non-self-supporting ladders were not used at an angle such than the horizontal distance from the top support to the foot of the ladder way approximately one-quarter of the working length of the ladder:  a) All Craft Construction LLC does not make sure that a non-self-supporting ladder was not used at an angle such than the horizontal distance from the top support to the foot of the ladder way approximately one-quarter of the working length of the ladder, on or about 9/19/23.   25-foot Werner Multi position pro ladder was used to access the roof that was about 20 feet heigh from the ground below.

29 CFR 1926.1053(b)(7): Slip resistant feet were used as a substitute for care in placing, lashing, or holding a ladder that is used upon slippery surfaces, including, but not limited to, flat metal or concrete surfaces that are constructed so they cannot be prevented from becoming slippery.  a) 120 Passaic Street Hackensack NJ 07601: An employee was using a 25-foot Werner Multi position pro ladder that did not had slip resistant feet to prevent the ladder from sliding resulting on an accident, on or about 9/19/23.  Ladder slide down causing the employee to sustained head injuries requiring surgery and intubation.
