29 CFR 1910.147(d)(2): The machine or equipment was not turned off or shut down using the procedures established for the machine or equipment: a) 460 East Main Street, Middletown, DE 19709 (Meat Department): On or about December 11, 2023, employees used a Hobart 6801/6614 meat saw without having or using established procedures to follow prior to servicing and maintenance (set up) of the saw. Employees were exposed to an amputation hazard while operating the saw without ensuring the necessary guards were being used in the process of cutting the large portions of meat.

29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): a) 460 East Main Street, Middletown, DE 19709 (Meat Department): On or about December 11, 2023, employees used a vertical band saw to cut meat with an unguarded blade, exposing employees to amputation hazards. An employee suffered an amputation while using the unguarded band saw after coming into contact with the blade during operation.
