At the service shop: On or about December 6, 2023,the employer exposed employees to struck-by and crushing hazards, in that, the employer failed to conduct annual inspections for the Yale 3 Ton Hoist (Serial #G3WA6532TJ) used by employees to lift lawnmowers and tractors that were being repaired. b. 29 CFR 1910.101(b): The in-plant handling, storage, and utilization of all compressed gases in cylinders, portable tanks, rail tankcars, or motor vehicle cargo tanks were not in accordance with Compressed Gas Association Pamphlet P-1-1965, which is incorporated by reference as specified in CFR 1910.6: a. On or about December 6th, 2023, in the tool room located at 5017 Skyland E Blvd, Tuscaloosa, AL 35405, a helium cylinder did not have a valve protection cap installed to protect the exposed valve. b. On or about December 6th, 2023, in the tool room located at 5017 Skyland E Blvd, Tuscaloosa, AL 35405, a helium compressed gas cylinder not in use were not stored in a manner to prevent the cylinders from being knocked over or damaged by-passing objects. In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
