29 CFR 1910.119(l)(1): The employer did not implement procedures to manage changes to process chemicals, technology, equipment, and procedures, and changes to facilities that affect a covered process: a) D10 regenerator on CCR The employer failed to conduct an MOC review for temporary repairs to the head flange of the catalyst regenerator, such as injection of sealant into collar, packing rope and wrap with banding to stop the leak.
