29 CFR 1910.147(c)(5)(i): Locks, tags, chains, wedges, key blocks, adapter pins, self-locking fasteners, or other hardware were not provided by the employer for isolating, securing, or blocking of machines or equipment from energy sources: Furnace Department: On, or about, December 4, 2023, the press block provided for Machine 10253, a Federal full revolution mechanical power press, could not be used when clearing debris from the die set in that it was intended to be placed in the die set and would block the area that needed to be cleaned. An employee suffered a partial finger amputation when the ram drifted down while the employee attempted to clear debris from the die set cavity.

29 CFR 1910.217(e)(3): Original and continuing competence of personnel caring for, inspecting and maintaining mechanical power press(es) was not insured: Furnace Department: On, or about, December 4, 2023, employees who cleared debris from the tensile test die set were not provided with training regarding safe work procedures to be followed. An employee suffered a partial finger amputation when the ram drifted down while the employee attempted to clear debris from the die set cavity.
