29 CFR 1926.1051(a): Stairways or ladders were not provided at all personnel points of access where there was a break in elevation of 19 inches (48 cm) or more, and no ramp, runway, sloped embankment, or personnel hoist was provided: Location: 1357 NH Route 3A, Bow, NH On or about December 6, 2023, employees installing roof sheathing were not protected from fall hazards where a ladder was not used for access. Employees climbed out of a skyjack scissor lift and onto the interior trusses to gain roof access.
