Menard, Inc. on or about December 1, 2023, at the location of 14502 Country Rd. 15 Holiday City, OH 43554: The employer did not ensure that employees who service and maintain the travel path of the "Layer Picker 2" were adequately trained in the skills required for the safe application, usage and removal of energy controls. Employees were not adequately trained to isolate and render energy controls safe prior to performing "lathe" clearing tasks.

29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6). a). Menard, Inc. on or about December 1, 2023, at the location of 14502 Country Rd. 15 Holiday City, OH 43554: The employer did not ensure established energy control procedures were done in sequence by employees tasked with servicing "Layer Picker 2." Employees accessing the picker area to remove "lathe" from the machine's path of travel, without the benefit of energy isolation, were exposed to crushing hazards.
