29 CFR 1910.215(b)(9): The distance between the wheel periphery and the adjustable tongue or the end of the peripheral member at the top exceeded one-fourth inch. In the maintenance/fabrication area, the tongue guard of the bench grinder was adjusted to over one-fourth inch. Employees were exposed to laceration hazards.

In the fabrication area of the maintenance shop, the drill press did not have a chuck guard attached for use by employees. Employees were exposed to point of operation and debris hazards.
