29 CFR 1926.21(b)(2):The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his/her environment to control or eliminate any hazards or other exposure to illness or injury: 410 Project, wiseman and TX HWY-151 intersection, San Antonio TX.: On or about December 5, 2023, and at times prior thereto, workers were exposed to fall hazards while performing capitol form platform work with unsecured sections of walking surfaces to the platform floor.

1926.503(a)(2)(iii) The employer did not assure that each employee has been trained, as necessary, by a competent person in the use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used. 410 Project, Wiseman and TX HWY-151 intersection, San Antonio, TX.: On or about December 5, 2023, and at times prior thereto, workers were exposed to fall hazards while climbing over the top rail of a capitol form platform from an elevated aerial lift, and without their fall harness properly secured.
