29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: On the north side of the construction site, employees gained access to the roof of a commercial building and the ladder used did not reach high enough to extend the side rails above the upper landing surface exposing the employees to fall hazards.

29 CFR 1926.1053(b)(4): Ladders were not used for the purpose for which they were designed. On the north side of the construction site, employees used a 12-foot A-framed ladder while folded to access and egress the roof of a commercial building exposing the employees to fall hazards. The ladder was not used as intended and designed by the manufacturer.
