Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):  On or about Decemeber7, 2023, during the roofing operations of a residential structure located at 7271 State Rd., Parma, Ohio 44134; the employer did not ensure that employees on the walking/working surface were protected from a falls of approximately 10 feet 6 inches and 8 feet 10 inches by utilizing guardrail systems, safety net system, or personal fall arrest system.
