1926.21(b)(2) The employer did not instruct each employee in the recognition and avoidance of unsafe conditions and the regulations applicable to his work environment to control or eliminate any hazards or other exposure to illness or injury. On or about December 4, 2023, at Structure 28, during crane operations adequate instruction was not provided with the crane operator exited the crane cab, stood on the boom truck platform, and tied a polypropylene rope to one of the structure legs. The employee suffered an injury in a fall.
