On or about December 4, 2023, at the above addressed jobsite, an employee performing ground cleaning activities below employees performing roofing operations was not wearing a hard hat to protect against falling materials.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems: a. On or about December 4, 2023, at the above addressed jobsite, employees performing commercial roofing operations, seven (7') feet nine (9) inches above the ground, were not protected from falls, thereby exposing the employees to fall hazards.
