On the roof of a residence, at a temporary worksite located at 3218 Green Street in Jacksonville, Florida 32244: On or about December 7, 2023, the employer exposed employees to fall hazards, ranging from 11 to 14-foot, in that fall protection was not utilized by employees while removing the existing roof system.

At the Front Left-side elevation of a residence, at a temporary worksite located at 3218 Green Street in Jacksonville, Florida 32244: On or about December 7, 2023, the employer exposed employees to 11-foot fall hazards, in that a portable extension-type ladder was utilized by employees to gain access to the roof as the ladder's side rails did not extend 3 feet above the upper landing surface (the roof).
