Employee training shall include at least:   (h)(3)(iii): The measures employees can take to protect themselves from these hazards, including specific procedures the employer has implemented to protect employees from exposure to hazardous chemicals, such as appropriate work practices, emergency procedures, and personal protective equipment to be used;    a) On or about 11/28/2023, employer did not ensure that employees trained to follow appropriate work practices established for filling DOT cylinders were following the established procedures in regards flammable gas cylinders for defective safety valves, leaks,  checking recertification stamps showing when each cylinders had been recertified prior to filling cylinders, utilizing the scale to ensure actual fill volume for each cylinder to prevent exposing employees to the potential of explosions caused by cylinder ruptures and leaking flammable gases.
