29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet above the upper landing surface to which the ladder was used to gain access: On or about December 7, 2023, employees doing roof work on a single-family residential house were exposed to fall hazards by using extension ladders that did not extend 3-feet above the landing of the roof. (a) Front of house: Werner 24-feet extension ladder, model no. D1328-2. Employees were exposed to 9-feet fall hazard. (b) Side of house: 16-feet extension ladder. No model number and serial number could be recognized. Employees were exposed to 9-feet fall hazard. (c) Side of the house. 28-feet extension ladder. No model number and serial number could be recognized. Employees were exposed to 9-feet fall hazard. (d) Back of the house. 14-feet extension ladder. Employees were exposed to 9-feet fall hazard.
