29 CFR 1926.403(b)(1): Employer did not ensure that electrical equipment is free from recognized hazards that are likely to cause death or serious physical harm to employees: a) On or about December 11, 2023, at the above addressed jobsite, on the 3rd floor an energized electrical box was removed from the surface where it was mounted and left suspended by flexible electrical cable using a nail, thereby exposing employees to electrical hazards.

Conductors entering boxes, cabinets, or fittings were not protected from abrasion, and openings through which conductors enter were not effectively closed. Unused openings in cabinets, boxes, and fittings were not effectively closed. a) On or about December 8, 2023, at the above addressed jobsite, on the 13th floor, 3 unused openings in the 480V temporary power electrical panel were not effectively closed, thereby exposing employees to electrical hazards.

29 CFR 1926.1052(b)(1): Except during stairway construction, foot traffic was not prohibited on stairways with pan stairs where the treads and/or landings are to be filled in with concrete or other material at a later date, the stairs were not temporarily fitted with wood or other solid material at least to the top edge of each pan. a) On or about December 8, 2023, at the above referenced job site, an employee performing labor operations, was exposed to pan stairs where the treads and/or landing were not filled in with concrete or other material, thereby exposing the employee to hazards associated with falls.
