29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee who might be exposed to fall hazards:   a) Construction Site, 6206 Marble Mill Place, Frederick, CO 80516: On or about December 05, 2023, as the controlling employer, Brookfield Residential (Colorado) LLC, did not ensure its subcontractor Manny C.M.G, protected their employees in that employees were exposed to fall hazards of approximately 12 feet to the ground below while installing asphalt shingles on the second story of a residential roof. The employer failed to provide training on items such as, but not limited to; nature of fall hazards in the work area; correct procedures for inspecting the fall protection systems to be used; use and operation of guardrail systems, personal fall arrest systems, safety net systems, warning line systems, safety monitoring systems, controlled access zones, and other protection to be used; limitations on the use of mechanical equipment during the performance of roofing work.
