29 CFR 1926.501(b)(4)(ii): Each employee on a walking/working surface was not protected from tripping in or stepping into or through holes by covers. (a) WVSDB Blue & Gold Building: On or about December 7, 2023, and times prior, an employee was using an 8-foot stepladder to secure HVAC hangers to ceiling beams in a room around multiple uncovered floor holes 4in. up to 57in. in depth, exposing the employee to trip and fall hazards.
