Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system: a) Roof: On or about December 8, 2023, employees cleaning work materials off the roof were exposed to fall hazards of approximately forty-five (45) feet due to a lack of a guardrail systems, safety net systems, or personal fall arrest system. ABATEMENT CERTIFICATION REQUIRED
