Employees were performing roofing operations at an elevated height approximately 19-21 feet above the surface, without any means of fall protection by way of guardrail systems, safety net systems, or personal fall arrest systems.

29 CFR 1926.1053(b)(1):Where portable ladders were used for access to an upper landing surface and the ladder's length allows, the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface being accessed: At a residential worksite located at 4367 Hideaway Dr, Lees Summit, Missouri. Employee was exposed to a fall hazard by using a ladder which extended approximately foot above the landing surface of the roof to which he gained access.
