29 CFR 1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b): On or about December 8, 2023, during the installation of roofing material on a multi-family residential home located at 30412 Center Ridge Road, Westlake, Ohio, the employer did not ensure employees working at heights greater than 6 feet (roof of the site) were protected from a fall hazards, by utilizing a guardrail system, safety net system and/or personal fall arrest system. Brito's Roofing Corp. DBA Brito's Roofing and Restoration, LLC was previously cited for a violation of this occupational safety and health standard , which was contained in OSHA inspection number 1557430, citation number 1 , item number 1, and was affirmed as a final order on 11/24/2021, with respect to a workplace located at 29292 Hampshire Place, Westlake, Ohio.

29 CFR 1926.1053(b)(1):Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access: On or about December 8, 2023, during the installation of roofing material on a residential home located at 30412 Center Ridge Road, Westlake, Ohio, the employer did not ensure the sides rails of the portable ladder used to access the roof (upper landing surface) of a multi-family residential home extended at least 3 feet above the roofs (landing surface). A metal extension ladder used to access the roof extended approximately 2 feet (2 rungs) above the roof landing surface. Brito's Roofing Corp. DBA Brito's Roofing and Restoration, LLC was previously cited for a violation of this occupational safety and health standard , which was contained in OSHA inspection number 1557430, citation number 1 , item number 2, and was affirmed as a final order on 11/24/2021, with respect to a workplace located at 29292 Hampshire Place, Westlake, Ohio.
