29 CFR  1926.20(b)(2):The employer did not initiate and maintain a safety program which provides for frequent and regular inspections of jobsites, materials, and equipment to be made by a competent person:  On or about December 11, 2023, the employer did not conduct frequent inspections to provide a workplace where any employee, worker, laborer, or mechanic employed in the performance of the contract were free from recognized hazards such as but not limited to (a-b):  a) Employees performing decking activities on the roof of a residential home were exposed to an unprotected fall hazard of six (6) feet or greater to the ground below.  b) An employee working at ground level without head protection was exposed to struck-by hazards while decking activities involving the transfer of material to upper levels were being performed directly overhead.

29 CFR  1926.100(a): Employees were not protected by protective helmets while working in areas where there was a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns:  On or about December 11, 2023, an employee working at ground level without head protection was exposed to struck-by hazards while decking activities involving the transfer of material to upper levels were being performed directly overhead.

29 CFR  1926.501(b)(10): Each employee engaged in roofing activities on low-slope roofs with unprotected sides and edges 6 feet or more above lower levels, was not protected from falling by guardrail systems, safety net systems, personal fall arrest systems, or a combination of warning line system and guardrail system, warning line system and safety net system, or warning line system and personal fall arrest system, or warning line system and safety monitoring system.  Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:     On or about December 11, 2023, employees engaged in decking activities were not provided fall protection and exposed to an unprotected fall hazard of approximately six (6) feet or greater to the ground below.
