29 CFR  1926.651(c)(2):A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22m) or more in depth so as to require no more than 25 feet (7.62m) of lateral travel for employees:  On or about December 12, 2023 at jobsite located at 1901 Broadway St. Toledo OH 43609, the employer Hillabrand and Sons Construction LLC did not ensure a safe means of egress such as a ladder was placed in the approximately 7 feet deep trench in which an employee was working. One employee was exposed to cave-in, struck-by and slip, trip and fall hazards and injury which can occur from the inability to exit the trench quickly in case of an emergency.
