29 CFR  1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems.  On or before December 12, 2023, employees on the east side of the construction site, employees engaged in roofing activities without fall protection were exposed to unprotected fall hazards of 11 feet.

On or about December 12, 2023, and at times prior thereto, employees were accessing a one story roof upper landing surface and did not extend the ladder 3 feet about the roof line exposing the employees to fall hazards.
