At a temporary single-family residential worksite located at 5040 McManus Road in Jacksonville, Florida 32210: On or about December 13, 2023, the employer exposed employees to head injury hazards, in that protective helmets, such as hard hats, were not utilized by employees who were at ground level, removing roofing debris as other employees worked from the roof and discarded roofing debris from the roof to the ground.
