29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section: a.) On or about November 16, 2023, the operator was exposed to pinch point and crushing hazards such as those created by reciprocating motions of the horizontal ram within the doser assembly of the Rethceif HC-2030 (Line 3), in that the Lock Out Tag Out procedure was not utilized.
