29 CFR  1910.133(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or injurious light radiation.  a) On or about December 13, 2023, at the bandsaw in the cut room, employees that operate the bandsaw were not protected by protective eye equipment where eyes may be exposed to flying particles or pieces of meat or bone.  b) On or about December 12, 2023, on the kill room floor, employees operating the splitting saw were not protected by protective eye equipment where eyes may be exposed to flying particles or pieces of meat or bone.  c) On or about December 13, 2023, in the storage room, cut room and kill room floor, employees mixing and spraying sanitation chemicals were not protected by protective eye equipment where their eyes may be exposed to corrosive liquid chemicals, including bleach or Nyco N601+.

29 CFR  1910.147(c)(4)(ii):  The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy: including, but not limited to specific statement of intended use of the procedure, specific procedural steps, and specific requirements for verifying effectiveness of energy control measures:  a) On or about December 13, 2023, employees performing servicing on the Variovac Optimus packaging machine were not protected from potential reenergization or hazards associated with stored energy by an energy control procedure meeting the requirements of this section. The energy control procedure developed by the employer for this machine did not outline the intended use of the procedure; the specific procedural steps for shutting down, isolating, blocking and securing the machine to control hazardous electrical and air pressure energy sources; specific procedural steps for the placement, removal and transfer of lockout devices and the responsibility for them; nor the specific requirements for testing the machine to determine and verify the effectiveness of energy control measures.
