29 CFR 1910.147(d): The established procedure for the application of energy control (the lockout or tagout procedures) did not cover the actions listed in and was not done in sequence as required by 29 CFR 1910.147(d)(1)-(6):  a) Graphic Packaging International, LLC, on or about December 7, 2023, at site location of 1300 S. Main St. Kenton OH: The employer did not ensure energy control application steps were implemented prior to the performance of a maintenance tasks on the Paper Cup Machine #103 (Model: PMC-1250).  A maintenance mechanic performing adjustments to the machine in an energized state was exposed to caught-in hazards.
