29 CFR 1926.452(o)(2)(iii): The supporting rope between the scaffold and the suspension device was not kept vertical and was not protected to ensure that it would not chafe at any point where a change in direction occurred:  a.  On or about December 13, 2023, at the above addressed jobsite, the boatswains' chair support rope was not protected from chafing along the edge of the roof, thereby exposing an employee performing exterior building washing activities to fall hazards and scaffold collapse hazards.

29 CFR  1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system:  a.  On or about December 13, 2023, at the above addressed jobsite, an employee performing exterior building washing activities from a boatswains' chair, approximately twenty-eight (28) feet above ground, was not protected by a personal fall arrest system, thereby exposing the employee to fall hazards.  Anfer Construction Inc. was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.451(g)(1)(i), which was contained in OSHA inspection number 1535759, citation number 1, item number 3, and was affirmed as a final order on December 8, 2021, with respect to a workplace located at 622 W. Willow St. in Chicago, Illinois.
