On or about December 13, 2023, at the above addressed jobsite, an employee performing exterior building washing activities from a boatswains' chair, approximately twenty-eight (28) feet above ground, was not protected by a personal fall arrest system, thereby exposing the employee to fall hazards.  Anfer Construction Inc. was previously cited for a violation of this occupational safety and health standard, 29 CFR 1926.451(g)(1)(i), which was contained in OSHA inspection number 1535759, citation number 1, item number 3, and was affirmed as a final order on December 8, 2021, with respect to a workplace located at 622 W. Willow St. in Chicago, Illinois.
