29 CFR  1926.102(a)(1): Employees did not use eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:   (a) 533 South 2nd Street, Lykens, PA, Main Roof, - Employees did not don safety glasses while using pneumatic nail guns to attach shingles to the roof of a townhome building and were exposed to flying debris and ricochet nails, on or about  December  14, 2023.  (b) 533 South 2nd Street, Lykens, PA, Back Roof, - Employees did not don safety glasses while using pneumatic nail guns to attach shingles to the roof of a townhome building and were exposed to flying debris and ricochet nails, on or about  December  14, 2023.  (c) 533 South 2nd Street, Lykens, PA, Front Porch Roof, - Employees did not don safety glasses while using pneumatic nail guns to attach shingles to the roof of a townhome building and were exposed to flying debris and ricochet nails, on or about  December  14, 2023.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR  1926.501(b)(13):Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   (a) 533 South 2nd Street, Lykens, PA, Main Roof, - Employees were performing residential roofing operations on a two-story residential roof,  approximately 25 feet above grade with an 6/12 roof pitch, without utilizing any means of fall protection, on or about , on or about December 14, 2023.  (b) 533 South 2nd Street, Lykens, PA, Back Roof, - Employees were performing residential roofing operations on a two-story residential roof,  approximately 18 feet above grade with an 4/12 roof pitch, without utilizing any means of fall protection, on or about , on or about December 14, 2023.  (c) 533 South 2nd Street, Lykens, PA, Front Porch Roof, - Employees were performing residential roofing operations on a residential front porch roof, on or about December 14, 2023.  Abatement certification and documentation required within 10 days after abatement date. The certification shall include a statement that abatement is complete, the date and method of abatement, and state that employees and their representatives were informed of this abatement. Abatement documentation shall include documents demonstrating that abatement is complete, such as evidence of the purchase or repair of equipment, photographic or video evidence of abatement or other written records.
