29 CFR 1910.95(d)(1): When information indicated that any employee's exposure equaled or exceeded the 8-hour time-weighted average of 85 decibels, the employer did not develop and implement a monitoring program. a) 3008 Penns Valley Pike, Centre Hall, PA 16828 - On or about October 27, 2023, employees were working in the Entr�e Room near equipment that was producing hazardous noise levels, and the employer had not conducted monitoring to determine the employee's 8-hour time, weighted average exposure. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.

29 CFR 1910.95(m)(3)(ii):The employer did not retain audiometric test records for the duration of the affected employee's employment as required by 29 CFR 1910.95(m): a) 3008 Penns Valley Pike, Centre Hall, PA 16828 - On or about October 27, 2023, an employees was working in the Entr�e Room near equipment that was producing hazardous noise levels, and the employee had received audiometric testing, however the employer did not maintain the audiometric testing records. Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states that employees and their representatives were informed of this abatement.
