On or about December 14, 2023, and at times prior thereto, the employer did not ensure employees completed training prior to allowing the employees to operating the forklift.

29 CFR 1910.184(c)(1): Slings that are damaged or defective shall not be used. On or about December 14, 2023, and at times prior thereto, alloy chain slings that were damaged or defective and missing safety latches were not used. 1. Three alloy steel chains used on the Gaffey 3 Ton Crane Ser No. 388-FWC did not have a safety latch and two of the chains equipped with latches did not fully close. 2. Two alloy steel chains used on the Gaffey 3 Ton Crane Ser No. 389-FWC did not have safety latches. The chain hooks were previously equipped with safety latches.
