29 CFR 1910.23(b)(13):Employees were carrying an object or load that could cause the employee to lose balance and fall while climbing up or down the ladder. On or about November 30, 2023 and at times prior, employees were exposed to a fall hazard while supporting a floor hatch door with one hand while descending a fixed ladder and unable to maintained three points of contact with the ladder. Actual Incident November 30, 2023, an employee was seriously injured while descending a fixed ladder when his hand holding the floor hatch open slipped causing the hatch to fall and strike the employee causing him to fall off the ladder and hit the floor below.

29 CFR 1904.39(a)(2):The employer did not report an in-patient hospitalization, amputation, or loss of an eye as a result of a work-related incident to OSHA within twenty-four (24) hours. On or about November 30, 2023 and employee was injured and hospitalized for a fall. The employer failed to report the in hospitalization within the 24 hour time frame allotted by this standard.
