29 CFR 1910.95(k)(2): The training program was not repeated annually for each employee included in the hearing conservation program: a) Production area - On or about 12/13/2023, and at times prior to, an employee working near injection and compression molding machines was exposed to continuous noise dose of 61.3%, or an equivalent of 86.4 dBA that exceed an 8-hour time-weighted average of 85 dBA. The employer failed to ensure all employees exposed to noise levels at or above action level of an 8-hour Time Weighted Average (TWA) of 85dBA were trained annually on noise hazards. The sampling was performed for 470 minutes during one shift on 12/13/2023; exposure calculations included a zero increment for the 10 minutes not sampled. b) Production area - On or about 12/13/2023, and at times prior to, an employee working near injection and compression molding machines was exposed to continuous noise dose of 61.6%, or an equivalent of 86.5 dBA that exceed an 8-hour time-weighted average of 85 dBA. The employer failed to ensure all employees exposed to noise levels at or above action level of an 8-hour Time Weighted Average (TWA) of 85dBA were trained annually on noise hazards. The sampling was performed for 430 minutes during one shift on 12/13/2023; exposure calculations included a zero increment for the 50 minutes not sampled. c) Production area - On or about 12/13/2023, and at times prior to, an employee working near injection and compression molding machines was exposed to continuous noise dose of 87.9%, or an equivalent of 89.0 dBA that exceed an 8-hour time-weighted average of 85 dBA. The employer failed to ensure all employees exposed to noise levels at or above action level of an 8-hour Time Weighted Average (TWA) of 85dBA were trained annually on noise hazards. The sampling was performed for 469 minutes during one shift on 12/13/2023; exposure calculations included a zero increment for the 11 minutes not sampled. d) Production area - On or about 12/13/2023, and at times prior to, an employee working near injection and compression molding machines was exposed to continuous noise dose of 117.8%, or an equivalent of 91.1 dBA that exceed an 8-hour time-weighted average of 85 dBA. The employer failed to ensure all employees exposed to noise levels at or above action level of an 8-hour Time Weighted Average (TWA) of 85dBA were trained annually on noise hazards. The sampling was performed for 452 minutes during one shift on 12/13/2023; exposure calculations included a zero increment for the 28 minutes not sampled.
