On or about December 15, 2023, an employee on the north side of the structure was conducting stucco operation using a scaffold with platforms that were not a minimum of 18 inches wide.

29 CFR  1926.451(e)(1):When scaffold platforms are more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from another scaffold, structure, personnel hoist, or similar surface shall be used.   On or about December 15, 2023 employees performed stucco work on a scaffold the employer did not provide a ladder for the employees to access the upper levels of the scaffold.

On or about December 15, 2023 employees performed stucco work on a scaffold more than 10 feet above the lower level  without the employer providing additional protection from falling hand tools, debris, and other small objects.

The program shall enable each employee to recognize the hazards of falling and shall train each employee in the procedures to be followed in order to minimize these hazards.  On or about December 15, 2023, an employee on the north side of the structure was conducting stucco operation on the roof without fall protection training prior to conducting the work.
