29 CFR  1926.501(b)(1): Each employee on a walking/working surface having an unprotected side or edge which was six or more feet (1.8m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest system.   On December 13, 2023, employees engaged in framing work on a commercial building without fall protection were exposed to unprotected fall hazards of approximately 9 feet to the ground below.

29 CFR  1926.1053(b)(1): The ladder extension was positioned such that deflection under a load, by itself, would cause the ladder to slip off its support.   On December 13, 2023, employees engaged in framing work on a commercial building use an extension ladder that did not have the side rails extended three feet over the upper landing surface were exposed to potential fall hazards greater than six feet.

On the construction site, the employer did not develop or provide fall protection training to the employees engaged in framing work to recognize and eliminate fall hazards when working over 6 feet.
