29 CFR 1910.178(a)(4): Modifications and additions which affect capacity and safe operation shall not be performed by the customer or user without manufacturers prior written approval. Capacity, operation, and maintenance instruction plates, tags, or decals shall be changed accordingly:  (a) At the plant: On or about 12/22/2023 and at times prior, the employer exposed employees to struck-by hazards, in that an  approved attachment was not used in the forklift while connecting small baskets that come from the oven areas and transport them over to the chemical line for cleaning.   In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
