29 CFR 1926.100(a):   Head Protection - Employees working in areas where there is a possible danger of head injury from impact, or from falling or flying objects, or from electrical shock and burns, shall be protected by protective helmets.  511 Moosup Pond Road: On or about December 14, 2023, Employer did not protect Employee(s) from accidental injury when Employee(s) were exposed to potential danger of head injury from impact, or from falling or flying objects due to having no protective helmets. Employee(s) were working without any head protection, and directly in front of and below other Employee(s) utilizing forced air equipment to dislodge debris from the roof above while performing roof removal-repair-replace activities.

29 CFR 1926.452(k)(5):   Scaffolding - Scaffold platforms shall not be bridged one to another.  511 Moosup Pond Road: On or about December 14, 2023, Employer did not protect Employees from accidental injury by ensuring that scaffolding platforms were not bridged one to another. Employees were utilizing scaffolding platforms which were assembled properly to perform roof repair-removal-replace activities 6-feet to 12-feet above ground.  1) 511 Moosup Pond Road - Front of Home: Scaffold bridged with tubular scaffolding equipment, 16-feet on left and 14' on right. 2) 511 Moosup Pond Road - Rear of Home: Scaffold bridged with tubular scaffolding equipment on residential porch.

29 CFR 1926.501(b)(13):   Fall Protection - Each employee engaged in residential construction activities 6 feet (1.8 m) or more above lower levels shall be protected by guardrail systems, safety net system, or personal fall arrest system unless another provision in paragraph (b) of this section provides for an alternative fall protection measure.   511 Moosup Pond Road: On or about December 14, 2023, Employer did not protect Employee(s) from accidental death or injury by providing Employee(s) engaged in residential construction activities 6-feet or more above lower levels, protective guardrail systems, safety net system, or personal fall arrest system. Multiple Employees were identified performing roofing activities 6-feet above ground and higher, without any fall protection systems.
