29 CFR 1910.212(a)(3)(ii): Point(s) of operation of machinery were not guarded to prevent employee(s) from having any part of their body in the danger zone(s) during operating cycle(s): Location: Johnny 5 Room On or about December 8, 2023, a running DC-8 delumper rotary valve was not guarded and two of the employee's fingertips were crushed and amputated when the employee's hand was under the hole at the base of the rotary valve.
