29 CFR 1926.501(b)(5): Each employee on the face of formwork or reinforcing steel was not protected from falling 6 feet (1.8 m) or more to lower levels by personal fall arrest systems, safety net systems, or positioning device systems: Site: 205 Java St in Brooklyn, NY. 6th Floor. a) On or about 12/15/2023, an employee was working on the formwork beams and fell 10 feet through the exposed opening below. The employee was not protected from falling by a personal fall arrest system, safety net system, or positioning device system.
