29 CFR  1910.28(b)(3)(i):29 CFR 1910.28(b)(3)(i):  The employer did not ensure that each employee is protected from falling through any hole (including skylights) that is 4 feet (1.2 m) or more above a lower level by one or more of the following methods listed in 29 CFR 1910.28(b)(3)(i)(A)-(D):  At a facility located at 911 North farm Road 123, Springfield, MO 65802, employees were exposed to fall hazards.  The employer exposed the employees to these hazards by allowing them to work on the roof of the facility, near skylights, without protecting the employees from falling through the skylights.  This hazard was most recently documented on or about 12/14/2023, when an employee fell through a skylight while working on the roof of the facility.

29 CFR  1910.30(a)(1):29 CFR 1910.30(a)(1): The employer did not provide training for each employee who uses personal fall protection systems or who is required to be trained as specified elsewhere in this subpart before any employee is exposed to a fall hazard. And/or the employer did not ensure employees are trained in the requirements of this paragraph on or before May 17, 2017.  At a facility located at 911 North farm Road 123, Springfield, MO 65802, employees were exposed to fall hazards.  The employer exposed the employees to these hazards by allowing them to work under circumstances that required the use of fall protection without first providing the employees training in regard to fall protection systems.  This hazard was most recently documented on or about 12/14/2023, when an employee fell through a skylight while working on the roof of a structure.

29 CFR  1910.1200(e)(1)(i):The employer did not compile a list of the hazardous chemicals known to be present using a product identifier that was referenced on the appropriate safety data sheet.  At a facility located at 911 North farm Road 123, Springfield, MO 65802, employees were exposed to hazards related to chemical exposure as the employer failed to compile a list of hazardous chemicals in use at the facility.
