29 CFR 1910.119(j)(4)(i): The employer did not perform inspection and tests on process equipment: (a) Maleic Anhydride Process, Vaporizer E-229: On December 19, 2023, the employer did not conduct an ultrasonic thickness test on Vaporizer E-229 containing butane that was required per the inspection plan. Failure to conduct inspections and tests on the vaporizer could result in loss of containment of butane from the vaporizer, resulting in a vapor cloud exposing employees to fire/explosion hazards.
