29 CFR 1910.29(b)(2)(i): The employer did not ensure that guardrails met the requirement that midrails are installed at a height midway between the top edge of the guardrail system and the walking-working surface:  On or about December 19, 2023, and at times prior thereto, upper working platforms on the structural foam machines used by employees during mold changeouts had guardrail systems with broken, bent, and/or missing midrails, exposing them to fall hazards greater than four (4) feet.

29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section:  On or about December 16, 2023, at times prior thereto and thereafter, machine-specific energy control procedures were not developed and utilized for the control of hazardous energy, exposing employees to the hazards of unexpected energization, or release of stored energy from machines such as, but not limited to, blow molding machines, structural foam machines, mold injection machines, and sterling machines. Employee exposures include the following instances (a-c):  a) Maintenance Technicians performing maintenance and servicing operations were exposed to hazardous energy sources such as, but not limited to, electric, hydraulic, pneumatic, and thermal.  b) Tool Setters performing servicing operations were exposed to hazardous energy sources such as, but not limited to, electric, hydraulic, pneumatic, and thermal.   c) Process Technicians performing servicing and cleaning operations were exposed to hazardous energy sources such as, but not limited to, electric, hydraulic, pneumatic, and thermal.
